Ek therapy jisme yeh samjha jaata hai ke hamare khayal hamare jazbaat aur rawayye ko kaise mutasir karte hain — aur un khayalon ko behtar banane se zindagi behtar hoti hai.
English Definition
Cognitive therapy is a structured, evidence-based form of psychotherapy developed by psychiatrist Aaron Beck in the 1960s. It is built on the principle that our thoughts — not events themselves — determine how we feel and behave. By identifying, examining, and reshaping unhelpful or distorted thinking patterns, cognitive therapy helps people experience lasting improvements in their emotional wellbeing and daily functioning.
Cognitive Therapy vs Psychodynamic Therapy vs Behavioural Therapy
Cognitive Therapy
Psychodynamic Therapy
Behavioural Therapy
Core focus
Thoughts and beliefs that drive emotion and behaviour
Unconscious patterns and past experiences
Observable behaviours and what reinforces them
Time orientation
Primarily present — with some exploration of how beliefs formed
Past and present — how early experience shapes now
Present — what is happening and how to change it
Structure
Highly structured — session agendas, thought records, exercises
Less structured — follows the client's associations
Structured — uses specific behavioural techniques
Roots & Origins
From Old English haelan, meaning "to cure, save, make whole, sound, and well," heal carries an older sense of restoration rather than mere repair. Read in that light, support around cognitive therapy and thought patterns can be framed not only as symptom management, but as a return toward wholeness - mental, emotional, bodily, and relational.

Detailed Explanation — The Cognitive Model
The foundation of cognitive therapy is deceptively simple: it is not what happens to us that determines how we feel, but what we think about what happens to us. Aaron Beck observed that people experiencing depression, anxiety, and other psychological difficulties were often trapped in patterns of automatic, distorted thinking — thoughts that felt completely true and completely beyond question, but which, on examination, turned out to be unreliable interpretations rather than facts.
Cognitive therapy teaches people to do something that most of us have never been shown how to do: to slow down, notice their thoughts, question them, and replace the distorted ones with something more accurate and more helpful. This is not about positive thinking or convincing yourself that everything is fine. It is about thinking more clearly — and discovering that clearer thinking genuinely changes how you feel.
Cognitive Therapy in Pakistan
The core principles of cognitive therapy map remarkably well onto Pakistani experiences of psychological distress. Many of the most common forms of suffering in Pakistan — persistent shame, catastrophic thinking about social judgment, deep-seated beliefs of worthlessness instilled in childhood, or the conviction that one is permanently unlucky or cursed — are precisely the kinds of cognitive distortions that cognitive therapy is designed to address. While the therapy itself originates in a Western clinical context, its tools are practical, flexible, and highly adaptable to the cultural and linguistic realities of Pakistani life. A growing number of therapists trained in Pakistan are now using cognitive approaches — often in Urdu — to work with clients across a range of difficulties.

Common Myths About Cognitive Therapy in Pakistan
"Yeh sirf positive sochne ke baare mein hai" (It is just about thinking positively)
"Mere khayal mere ikhtiyaar mein nahi hain, toh therapy kaise kaaam karegi" (My thoughts are not in my control — so how can therapy help?)
"Itni mushkilaat hain meri zindagi mein — sirf soch badlne se kya hoga" (My problems are real — just changing my thinking won't fix them)
"Yeh sirf parhe likhe logon ke liye hai" (This is only for educated people)
"Agar main apne khayalon ko challenge karna shuru kiya toh aur zyada confused ho jaunga" (If I start questioning my thoughts I will only become more confused)
"Cognitive therapy mein bas homework diya jaata hai" (Cognitive therapy is just about being given homework)
Core Concepts in Cognitive Therapy
Automatic Thoughts
These are the rapid, spontaneous thoughts that flash through our minds in response to situations — often so fast we barely notice them. In cognitive therapy, you learn to slow down and catch these thoughts before they shape your emotional response. For example: a friend does not reply to a message and you automatically think "they must be angry with me" — leading to anxiety or withdrawal. Cognitive therapy asks: is that thought accurate? What else might explain it?
Cognitive Distortions
Cognitive distortions are systematic errors in thinking — patterns of inaccuracy that the mind falls into habitually. Common ones include:
All-or-nothing thinking — "If I am not perfect, I am a complete failure"
Catastrophising — "If this goes wrong, it will be a disaster I cannot recover from"
Mind reading — "I know they think badly of me"
Personalisation — "Everything that goes wrong is my fault"
Overgeneralisation — "This always happens to me — it will never get better"
Emotional reasoning — "I feel worthless, therefore I must be worthless"
Filtering — focusing entirely on the one negative detail while ignoring everything positive
Core Beliefs
Beneath automatic thoughts and distortions lie deeper, more foundational beliefs about the self, others, and the world — formed early in life, often in response to painful experiences. Common core beliefs include "I am not good enough," "I am unlovable," or "The world is not safe." Cognitive therapy works gradually toward identifying and reshaping these beliefs, not just the surface thoughts that spring from them.
The Thought Record
One of the central tools of cognitive therapy — a structured way of writing down a difficult situation, the automatic thought it triggered, the emotion it produced, and then examining the evidence for and against that thought before arriving at a more balanced perspective. Simple in concept, but genuinely transformative in practice.
Related Approaches
Cognitive Behavioural Therapy (CBT)
The most widely known development of cognitive therapy — combining Beck's cognitive model with behavioural techniques. CBT addresses both the thoughts and the avoidance behaviours that maintain psychological difficulties. It is currently the most extensively researched form of therapy in the world.
Rational Emotive Behaviour Therapy (REBT)
Developed by Albert Ellis — a related approach that focuses specifically on identifying irrational beliefs and replacing them with more rational ones. More confrontational in style than Beck's cognitive therapy.
Acceptance and Commitment Therapy (ACT)
A newer development in the cognitive tradition — rather than challenging and changing thoughts, ACT teaches people to observe their thoughts with detachment, accept what cannot be changed, and commit to action aligned with their values.
Mindfulness-Based Cognitive Therapy (MBCT)
Combines cognitive therapy with mindfulness practices — particularly effective for preventing relapse in recurrent depression. Teaches people to notice when they are slipping into depressive thinking patterns before they take hold.
How Long Does Cognitive Therapy Take?
One of cognitive therapy's most significant advantages is its relatively short timeframe. Most people working on a specific concern — such as depression, a phobia, or social anxiety — complete a meaningful course of cognitive therapy in 12 to 20 sessions. More complex presentations, such as long-standing low self-worth or OCD, may benefit from a longer course of treatment. Progress in cognitive therapy tends to be visible and measurable — both the therapist and client can usually see clear change over time, which makes it easier to know when enough has been done.
Is Cognitive Therapy Right for You?
Cognitive therapy tends to work best for people who:
Are dealing with depression, anxiety, or persistent unhelpful thinking patterns
Are looking for structured, practical support with clear techniques to apply
Are willing to engage between sessions — reflecting on thoughts, practising exercises, and bringing observations back to the next session
Want to understand the connection between their thinking, their feelings, and their behaviour
It may be less suitable as a standalone approach for people dealing with very deep-rooted trauma, complex personality difficulties, or grief — though it often forms a valuable part of a broader treatment plan.
When to Reach Out
If you recognise yourself in the thinking patterns described here — the inner critic that never quietens, the catastrophic spiral that takes over under stress, the belief that you are somehow fundamentally not enough — cognitive therapy may be exactly what you need. You can book a session with The Healing Lounge Pakistan's therapists to find out whether this approach is the right fit for you.
