Ek dimagi wirsa jisme insaan ka tawajjo qayam rakhna, khud ko rokna, aur apne kaam organize karna mushkil hota hai — yeh koi laaparwahi ya badtameezi nahi, balkay dimaag ki alag wiring hai.
English Definition
ADHD — Attention Deficit Hyperactivity Disorder — is a neurodevelopmental condition, meaning it originates in how the brain develops, and is present from birth. It affects a person's ability to regulate attention, control impulses, and manage behaviour and organisation across different settings. It is not caused by bad parenting, lack of discipline, or low intelligence. It is a recognised medical condition, among the most researched in the field of mental health, and it responds well to the right combination of support and treatment.
ADHD vs Poor Discipline
In Pakistan, ADHD is almost always read as a parenting failure or a character flaw. This table addresses that directly.
ADHD
Poor Discipline / Laziness
Consistent across all settings — home, school, relatives, everywhere
Situation-dependent — the child behaves differently based on context or consequences
Present from early childhood, not a sudden change
Often develops in response to specific environments or life changes
The person genuinely cannot sustain attention even when they want to
The person chooses not to engage when unmotivated
Affects tasks the person cares about too, not just things they dislike
Roots & Origins
From Old English haelan, meaning "to cure, save, make whole, sound, and well," heal carries an older sense of restoration rather than mere repair. Read in that light, support around ADHD symptoms and executive-function struggles can be framed not only as symptom management, but as a return toward wholeness - mental, emotional, bodily, and relational.
Looking for ADHD assessment or support?
Clinical psychological support can help clarify ADHD-like patterns, executive-function difficulties, and the next steps for assessment or therapy.
Good support for ADHD symptoms and executive-function struggles is not just about a label. It is an integrative journey that brings together clear language, practical next steps, and a felt sense of safety.
Acceptance of current state
“Healing is not the absence of the wound, but the ability to carry it with gentleness.”

The person is generally unbothered by their behaviour
Does not respond to punishment alone
Usually responds to consistent consequences and boundaries
Neurological — visible in brain scans and genetic research
Behavioural — shaped by environment and learned patterns
Detailed Explanation
ADHD is one of the most common neurodevelopmental conditions in the world, yet in Pakistan it remains almost entirely unrecognised. Children are labelled shareer, awalul, aalas — naughty, stupid, lazy — and punished repeatedly for behaviour they are neurologically unable to simply stop. Parents are blamed for not raising them properly. Teachers give up on them. And the child, receiving no accurate explanation for why they keep failing despite trying, internalises the verdict: that something is fundamentally wrong with them as a person.
This is not a small matter. Years of being misread, punished, and shamed for a neurological difference — without anyone ever naming it correctly — does profound damage to a person's self-worth, their relationship with learning, and their sense of what they are capable of. Many adults in Pakistan are living with undiagnosed ADHD right now, having spent their entire lives believing they are simply not trying hard enough.
ADHD is not a deficit of intelligence. Many people with ADHD are highly creative, intensely curious, and capable of extraordinary focus when engaged — which is exactly why the condition is so frequently dismissed.
How Common is ADHD in Pakistan?
Global prevalence estimates suggest that ADHD affects approximately 5-7% of children and 2-5% of adults. Applied to Pakistan's population, this means millions of people are affected — the vast majority without any diagnosis, support, or understanding of what they are experiencing. Research from Pakistani schools indicates high rates of attention and hyperactivity difficulties among students, most of whom are managed through punishment rather than appropriate intervention.
Symptoms / Signs
ADHD presents differently in children and adults, and differently across its three types. Understanding both dimensions matters.
In Children
Inattentive Signs
Loses focus easily, especially during tasks that require sustained mental effort
Makes frequent careless mistakes in schoolwork despite being capable
Appears to not listen even when spoken to directly
Does not follow through on instructions or complete tasks — starts, then trails off
Has difficulty organising tasks, belongings, and time
Avoids or strongly resists tasks requiring prolonged concentration
Loses things constantly — stationery, books, belongings
Easily distracted by the smallest external stimulation
Forgetful in daily routines
Hyperactive and Impulsive Signs
Fidgets, squirms, or cannot remain seated when expected to
Runs or climbs in situations where it is not appropriate
Cannot engage in activities quietly
Always "on the go" — as if driven by a motor
Talks excessively
Blurts out answers before a question is finished
Cannot wait their turn — in games, in conversations, in queues
Interrupts or intrudes on others constantly
In Adults
ADHD in adults looks different. The obvious physical hyperactivity often reduces — but the internal experience intensifies.
Chronic difficulty starting tasks, even urgent ones — known as task initiation failure
Hyperfocus on interesting things for hours, inability to engage with anything else — this is a symptom, not proof the ADHD is fake
Perpetually late, missing deadlines, forgetting appointments despite caring about them
Relationship strain from forgetfulness, not listening, impulsive responses
A lifetime of underachieving relative to actual intelligence and potential
Chronic inner restlessness — described as a buzzing or a never-quiet mind
Emotional dysregulation — intense, fast-moving emotional reactions that feel impossible to manage
Difficulty managing finances, household responsibilities, and long-term planning
History of job changes, unfinished projects, and half-completed goals
ADHD in Girls — The Most Overlooked Group
Girls with ADHD are diagnosed far later than boys — often not until adulthood, if at all. This is because the condition presents differently in girls and because our picture of ADHD is built almost entirely around boys.
Girls with ADHD are more likely to have the inattentive type — they do not disrupt the classroom. They sit quietly, they daydream, they appear "lost in their own world." They are called sensitive, scatter-brained, or emotional. They develop sophisticated masking strategies — working twice as hard as everyone else to appear normal — which hides the condition further and leads to burnout, anxiety, and depression.
In Pakistan, where girls are already under intense pressure to be well-behaved, composed, and academically successful, an ADHD girl who is struggling is almost never identified as someone who needs support. She is labelled careless or not trying hard enough — and she believes it.
Types of ADHD
Predominantly Inattentive Presentation
Difficulty primarily with focus, organisation, and follow-through. Less obvious externally — the person appears "zoned out" or dreamy rather than disruptive. More common in girls. Most frequently missed.
Predominantly Hyperactive-Impulsive Presentation
Difficulty primarily with staying still, controlling impulses, and waiting. More externally visible. The classic image most people associate with ADHD.
Combined Presentation
Significant difficulties with both attention and hyperactivity/impulsivity. The most common presentation overall.
The Hyperfocus Paradox — "But They Can Focus on Video Games"
This is one of the most common reasons ADHD is dismissed in Pakistan — and it deserves a direct answer.
ADHD is not an inability to focus on anything. It is an inability to regulate focus — to direct and sustain attention on demand, regardless of interest level. The brain with ADHD is driven heavily by interest, novelty, challenge, and urgency.
Video games, certain subjects, creative activities — these provide instant, constant feedback and stimulation that allows focus to lock in. Homework, routine tasks, and administrative work do not. This is not a choice. The ADHD brain is not pretending when it cannot focus on the latter — it is experiencing a genuine neurological barrier. Hyperfocus on preferred activities is actually listed as a feature of ADHD, not evidence against it.
Causes / Why It Happens
ADHD is not caused by too much screen time, sugar, poor parenting, or lack of discipline. It is a neurodevelopmental condition shaped by:
Genetics — ADHD is highly heritable; it runs strongly in families. If a child has ADHD, there is a good chance at least one parent does too — often undiagnosed
Brain structure and chemistry — Differences in the prefrontal cortex (responsible for planning and impulse control) and in dopamine regulation are consistently found in people with ADHD
Prenatal factors — Premature birth, low birth weight, and exposure to alcohol, tobacco, or certain substances during pregnancy increase risk
Early adversity — Severe early stress or trauma does not cause ADHD but can worsen its symptoms significantly
Treatment for ADHD
ADHD is best managed through a combination of approaches. No single element is sufficient on its own.
Psychoeducation — Understanding the condition clearly is itself transformative, for the person and their family. Much of the damage done by ADHD comes from misunderstanding it.
Behavioural strategies — Structured routines, external organisation systems, breaking tasks into small steps, and environmental modifications that reduce friction
Therapy — CBT adapted for ADHD addresses the shame, avoidance, and self-defeating patterns that develop over years of misunderstanding. Coaching is also highly effective for adults.
School accommodations — Extended time, preferential seating, reduced distraction environments, and written instructions significantly improve academic outcomes. These are practical adjustments, not unfair advantages.
Medication — Stimulant medications such as methylphenidate are among the most studied and effective treatments in all of psychiatry. They work by regulating dopamine in the brain. They do not sedate, dull personality, or create dependency when used correctly under medical supervision. In Pakistan, access to these medications remains very limited, and a psychiatrist's assessment is needed.
Family support — Parents and caregivers need guidance on how to respond to ADHD behaviour in ways that support rather than shame. Consistent, low-emotion structure helps far more than punishment.
A Note to Parents Being Blamed
If your child has ADHD and you have been told it is because of how you raised them — that is not accurate. ADHD is neurological. You did not cause it. What you can do now is learn about it, advocate for your child to receive proper assessment, and shift from a discipline-only approach to one that combines structure with genuine understanding. That shift changes everything.
When to Seek Help
Seek a professional assessment if:
A child is consistently struggling at school despite evident intelligence and effort
Behaviour has been described as disruptive, inattentive, or impulsive across multiple settings — home, school, and social situations
An adult has struggled their entire life with organisation, follow-through, relationships, and focus, and recognises themselves in the descriptions above
Shame and a sense of personal failure have accumulated over years around these difficulties
Anxiety or depression are present alongside these symptoms — co-occurring conditions are very common in undiagnosed ADHD
A proper assessment by a child psychiatrist, clinical psychologist, or ADHD-informed professional is the necessary first step. ADHD is not something a person grows out of by being pushed harder — it is something that becomes manageable when it is finally understood.
